will be completed with latin characters – no special character.
Example: Regional Hospital Cluj Napoca

Code for schemes:

• Clinical chemistry – CC
• Immunology – IM
• Specific proteins – SP
• Hematology – HE

Number of participation:

How many times they will participate in one year per scheme.
For example in case of monthly schemes it should be written 12.

Instrument:

Name of the instrument they use to perform the analyzes. In case of multi instrument report: write the name of all instruments. For multi instrument participation, the client has to pay separately for each instrument.

Referral name:

Name of laboratory personal if it is required to be written on the Certificate of participation.
